These Terms apply to your use of our website and related online services. Clinical services are additionally governed by separate intake agreements, consent forms, and financial policies provided at the time of treatment.
2. About the Practice
WholePath Psychiatric Services is an outpatient psychiatric practice providing compassionate, evidence-based mental health care through telehealth and in-person services for children ages 6+, adolescents, and adults.
Services may include psychiatric evaluations, medication management, psychotherapy, telepsychiatry appointments, Spravato (esketamine) therapy, collaborative treatment planning, and ongoing psychiatric care, subject to provider discretion and clinical appropriateness.
3. No Provider-Patient Relationship Created by Website Use
Use of this website, including submitting contact forms or appointment requests, does not establish a provider-patient relationship.
A provider-patient relationship is formed only after completion of required intake documentation, clinical evaluation, and mutual agreement to initiate treatment.
If you are experiencing a psychiatric or medical emergency, do not rely on this website. Call 911, call or text 988, or proceed to the nearest emergency room.
4. Services Provided
Psychiatric Services
Services may include:
- Initial psychiatric evaluations
- Medication management and monitoring
- Psychotherapy
- Telepsychiatry appointments
- In-person psychiatric appointments
- Spravato (esketamine) therapy
- Collaborative treatment planning
- Ongoing psychiatric follow-up care
All services are provided in accordance with applicable laws, professional standards, and provider judgment.
Age Groups Served
Services are available for children ages 6+, adolescents, and adults, subject to provider discretion, clinical appropriateness, and applicable legal consent requirements.
5. Appointment Policies
Scheduling
Appointments are scheduled in advance and subject to provider availability. An initial evaluation may be required prior to ongoing treatment.
Telehealth services may require separate informed consent documentation prior to treatment.
Cancellation Policy
At least 24 hours' notice is requested for appointment cancellations. Failure to provide adequate notice may result in a cancellation fee.
Repeated missed appointments, non-compliance with treatment recommendations, or non-payment may result in discharge from the practice at provider discretion.
6. Payment and Insurance
Insurance
Insurance participation and coverage vary by provider network and individual policy. Patients are responsible for verifying benefits and coverage prior to receiving services.
Financial Responsibility
- Copayments, deductibles, and coinsurance amounts are due at the time of service.
- Self-pay patients are responsible for full payment at each visit.
- Outstanding balances may result in suspension or termination of services.
- Patients remain financially responsible for services rendered regardless of insurance reimbursement outcomes.
7. Medication Management
Prescription Policies
- Controlled substances may require ongoing monitoring and follow-up appointments.
- Medication changes require appropriate clinical evaluation.
- Early refills are generally not provided.
- Lost or stolen medications must be reported promptly and may require supporting documentation.
- Certain advanced treatments, including Spravato (esketamine) therapy, may require additional screening, monitoring, consent forms, and observation protocols.
Patient Responsibilities
Patients agree to:
- Take medications as prescribed
- Report side effects or concerns promptly
- Maintain scheduled appointments
- Provide accurate and complete medical information
8. Emergency Services
WholePath Psychiatric Services does not provide 24/7 emergency psychiatric or medical coverage.
In case of emergency:

- Go to the nearest emergency room
- Call or text 988 Suicide & Crisis Lifeline
- Contact local emergency or crisis intervention services
9. Discharge Policy
We reserve the right to terminate the provider-patient relationship in accordance with applicable laws and ethical standards, including but not limited to:
- Non-compliance with treatment recommendations
- Repeated missed appointments
- Inappropriate, abusive, or threatening behavior
- Non-payment of fees
- Clinical determination that services are outside the scope of practice
When appropriate, reasonable notice and referral options will be provided.
10. Limitations of Service
We do not provide:
- 24/7 emergency psychiatric or medical services
- Inpatient psychiatric treatment
- Emergency room services
- Court-ordered evaluations
- Disability evaluations
- Services outside the scope of outpatient psychiatric care
11. Website Use
Acceptable Use
You agree to use this website for lawful purposes only, including legitimate healthcare inquiries and appointment-related communications.
You agree not to:
- Attempt unauthorized access to systems or patient data
- Interfere with website functionality
- Submit false or misleading information
- Engage in harassment or abusive behavior
Prohibited Activities
Prohibited conduct includes, but is not limited to:
- Introducing malware or malicious code
- Attempting to gain unauthorized access to confidential information
- Commercial exploitation of website content without permission
12. Intellectual Property
